Mail, Fax or Email the following reports throughout your internship to your
advisor on a weekly or biweekly basis as indicated below.
The sheets below were adapted from the booklet which provides 12 weeks of report forms. The forms listed below were rearranged to better complete the 15 week term. Students are permitted to use either the booklet or this website as a source of
internship report forms.

After the completion of your internship, we ask that you return to
Morrisville to present your experiences to students and faculty. About 4 to
6 weeks before the end of the semester you will be contacted by email to
schedule your presentation. Following
are some suggested topics to help you develop your talk.

- Your Responsibilities and how they may have changed during the internship.
- Describe your typical work day
- Describe your activities and any shows or and specific event in which you participated as part of your internship?
- What did you learn?
- Were you offered a job?
- What did you learn NOT to do?
- How has this experience changed your plans for the future?
- What did you learn about working with clients
- What did you learn about working with veterinarians or other professionals.
- Would you recommend this to other intern students?
- Has this helped solidify what you have learned at Morrisville? How?
- What can Morrisville do to improve its education of students before sending them out in industry?

You must submit the following to your faculty representative before 9AM on the day of your presentation.

Signed worksheet for learning objectives (the triplicate form)
2.
Final employer evaluation
(the final page of your internship guidelines booklet).3. Paper to hand in (if you prefer, you may hand in a printout your
Powerpoint presentation)4. In addition, many students who have completed
online report forms and emailed them to their advisor, provide a folder
with the completed forms.
